Only the exact information required by Worker Protection Standard (WPS) passes through to the pink page. Heavy-duty pad has 50 forms and a built-in separator page to prevent transfer to other pages. 11-3/4”L X 8-1/2”W X 5/8”D pad is 3-hole punched for binder storage. This form's layout is excellent. The form, when filled out completely, provides instructions to the mixer and applicator, then allow the applicator to record the pertinent information post application. The form is in triplicate. The third form has everything blocked out except for the necessary record keeping items for pesticide applications.
