I've had to do some realignments on each packets because the forms are not cut the same. Was not able to use and a waste of money.

Form itself looks and feels great. Took a few tries to get our printer to line up the characters inline with the form due to the margin issues. As far as I know, no claims have been returned. No major complaints from us!
