The package arrived and upon inspection and testing, the form is not properly centered on the page. The cut registration marks are clearly visible. Poor QA on the product. Unusable as is. Attempting to return.

We have used the first ream of our UB04 forms and all seemed fine. We are on to our second ream and noticed that when we printed on them that things did not line up. Upon further inspection we see that his whole ream is unusable. We have set it to the side and moved on to ream 3 of which seems like it will work. I am not including an image here due to PHI on the forms. We will not be asking for a refund due to the age of our order (2/2019), even though we anticipate using more forms in 2020 we will just have to reconsider which vendor we use when making the next purchase.
